HIV/AIDS Continuum of Care
The main principles underlying the HIV/AIDS Continuum of Care (CoC) model are a strong and consistent plan, local ownership, and links both between different services in the referral hospital and between the community and the referral hospital. These principles have been the
foundation for an integrated provision of services including: (1) voluntary confidential counseling and testing (VCCT) services, linking prevention and care; (2) community services (home-based care (HBC) teams and people living with HIV/AIDS (PLHA) support groups); (3) health facility-based care (opportunistic infection (OI) treatment and antiretroviral therapy (ART) for adults and children; tuberculosis (TB)/HIV care and treatment; prevention of mother-to-child transmission (PMTCT), and laboratory and pharmacy services); and (4) support centres and group activities.
